The Process of CCC Alteration Certification Application-Updating Standards for Ballasts CCC Certification

1.Fill in 'CCC Alteration Application
2.Our office will check all documents,if they are right,we will fax or e-mail list of documents for CCC alteration for updating standards for ballasts and related notes.
a.Chinese/English original copy of CCC certificates with old version standards GB19510.1-2004,GB19510.4-2005,GB19510.9-2004 and GB19510.10-2009
b.'CCC Alteration Application Form'
c.Product description reports for ballasts(standard specification)
d.Copy of latest CCC certification factory inspection/follow-up inspection report(including corrective reports for non-conformities)
e.OEM processing agreement(need to submit if the applicants of CCC certification are not the factory).
f.Copy of business license,organization code certificate of CCC certificate holder
h.Other documents need to be submitted for CCC alteration application for updating standards for ballasts.
(4 copies of documents above with applicant seal),and send to our office.
3.After rechecking the received documents of CCC alteration application for updating standards for ballasts,we will send 'CCC certification sample sending notice' to applicants(by fax or E-mail),and inform the applicants to send samples as required.
4.We will send 'CCC charge notice for CCC alteration application' to applicants(by fax or E-mail) after confirming the samples have been sent as required.
5.Applicant should arrange payment according to 'CCC notice charge for CCC alteration application' and fax back 'remittance slip' to:8621-51095580-807 or e-mail to ccc#china-ccc-certification.com.
6.After we confirm that payment has entered to certain account,CCC alteration type test for updating standards for ballasts will begin.
7.Comprehensive assessment will begin after passing CCC alteration type test.
8.New CCC certificate of same certificate No.with new version standards GB19510.1-2009, GB19510.4-2009 and GB19510.9-2009 will be issued after passing CCC alteration assessment.
